Thank you for your interest in working through your accreditation with the Healthcare Quality Association on Accreditation.

Your next step is to sign-up and register to activate your HQAA account and to give the information necessary to develop your personalized, web-based Workroom. You will be completing an application, completing a credit card information page, offered Terms of Use for the web site, (it is highly recommended that you print off the “Terms”), given pricing information for the application and for the use of My Workroom TM , given a link to set your password and given access to a tutorial to maximize your Workroom usage.

The cost of the application is $200.00. HQAA will accept all major credit cards for this fee. You will receive a personalized workroom of standards that are applicable to your company accessible from any computer that is Internet capable.

Before you begin the application you will need to have the following information:

• Tax Identification Number
• Gross reported revenue for your company, rounded to the thousands
• Address and phone number for all sites to be accredited
• Name and title of person responsible for accreditation
• 855 or 855-S application form (information from form must match HQAA application
• Medicare Provider Number(s) for all sites
• National Provider Identifier (s) for all sites
• Medicaid Provider Number(s) for all sites
• Credit card (for application fee and all subsequent Workroom fees)
• Member number to VGM & Associates, if applicable
• If you have been given a referral code, be prepared to insert it in the application form

You should be prepared to identify the types of services that your company provides, as well as for each branch office also, if applicable. You will be asked to write a summarization and brief description of your company; for example, year of incorporation, business model, your delivery of service methods, service area, etc. This description will be kept on file and used with your permission with your picture on the HQAA Web Site after you are accredited.
